Thank you so much for sharing your excellent tips and pointers to those of us who share your profession with you! I love your "Session Summary Note" forms but I'm wondering how exactly you file them after you fill out one at your first, official intake session with a student? I watched your video and you explained how you fill them out very well, however I'm a little confused on what you do with them afterwards? Do you file them in the student's individual file that you keep locked up, or do you keep each one on each student in your "Session Summary" binder? Also, do you start another one each time you meet with the same student?

To answer your filing question, I have a folder for each student, each school year. It has their name, grade, and teacher on the tab. Everything I do with that student that year goes in that folder. This way, I can keep track of the students I see year-to-year. These files go in a locked cabinet.
I start a new form for each individual session because it gives different information (date, time spent with the student, session #).
